We are happy to see you whether you are being referred to us by your primary physician or you scheduled an appointment yourself.

 

Though our physicians and nurses may often be with other patients, if you leave your name, number, and a brief description of your reason for calling, we will return your phone call as soon as possible.

 

If you are unable to keep your scheduled appointment, please notify the office at least 48 hours in advance to reschedule. This allows other patients to use your time slot.


Emergency Phone Calls

We recognize that emergency situations may arise and we will do our best to respond to your problem promptly.

 

In the event of a severe situation, go immediately to the emergency department of the hospital or call 911.

 

If the situation is not that severe but one where you wish to speak to a physician, please call our office at 402-441-5600 during regular office hours, or call our answering service at 402-473-3813 after office hours. The answering service will forward messages to the physician on call.

Insurance and Medicare

As a new patient, please bring your insurance cards with you at the time of your appointment so we can make a copy for our records. If your insurance coverage changes, you must notify our office and allow us to copy your new insurance cards.

 

As a courtesy, we will directly file commercial insurance forms when you provide us with all necessary insurance information.

 

After receiving notification from your insurance company, we will bill you for the remaining balance due. Balances are due 30 days following the receipt of the statement. If you are unable to make payment in full, contact our office to set up payment arrangements.

 

Each insurance company has its own policies regarding payment. You should check with your insurance company to determine whether you need authorization for treatment, pre-certification, or a second opinion. It is absolutely necessary that you inform our office staff of this. Failure to do so may result in non-payment from your insurance company. You may then be held responsible for any balance incurred.

Prescription Refills

If you need a refill of a medication you are currently taking, please contact your pharmacist several days before you run out of your medication.

 

If an authorization to refill your medication is needed, your pharmacist will contact our office and get the required information.

All of our records are kept in strict confidence and will be released only with your written permission.

Fees and Payments

The fee for your initial visit will vary depending on the complexity of your problem. There is a fee for your examination, and any additional procedures that may be required during the course of your care.

 

You are expected to pay any co-pay, deductible, or co-insurance at the time of your service.

 

For your convenience, we accept cash, check, Visa, and MasterCard. You may arrange a payment plan as necessary for large

account balances.
